TERMS & CONDITIONS – KAECYN BEHAVIORAL HEALTH SERVICES

🚨 EMERGENCY NOTICE
This is not a crisis service. If you are experiencing a mental health emergency, call 988, dial 911, or go to your nearest emergency room immediately.


1. ACCEPTANCE OF TERMS
By accessing or using our website, services, scheduling system, telehealth platform, or any related content, you agree to comply with and be bound by these Terms & Conditions. If you do not agree, discontinue use immediately.


2. ELIGIBILITY REQUIREMENTS
To use our services, you must be:

  • At least 18 years old, OR
  • A parent/legal guardian requesting services for someone under 18
    By using our services, you affirm that you meet these requirements and provide accurate information.

3. NO CRISIS SERVICES
We do not provide emergency or crisis services. We cannot provide immediate assistance for emergencies.


4. NO PROVIDER-PATIENT RELATIONSHIP THROUGH WEBSITE USE
Using this website, sending messages, or submitting forms does not establish a therapeutic relationship. A provider-patient relationship only begins after:

  • Formal acceptance as a client
  • Completed intake process
  • Signed documentation and consent forms
  • Services officially begin

5. ACCURACY OF INFORMATION
We make reasonable efforts to provide accurate information, but we cannot guarantee completeness or reliability. Content may change at any time.


6. SCHEDULING AND APPOINTMENTS
Using scheduling tools doesn’t guarantee an appointment until confirmation is received. You agree to:

  • Arrive on time (virtual)
  • Provide accurate information
  • Follow cancellation policies
  • Use telehealth platforms responsibly

7. TELEHEALTH SERVICES
By using telehealth, you acknowledge:

  • You must be in a state where the provider is licensed
  • You’re responsible for a stable internet connection
  • You understand electronic communication privacy limitations

8. PRIVACY & CONFIDENTIALITY
We comply with HIPAA and state regulations. See our separate Privacy Policy for details on information collection, use, and protection.


9. COOKIES & TRACKING TECHNOLOGIES
The website may use cookies to improve the user experience. You may disable cookies through browser settings.


10. INTELLECTUAL PROPERTY
All website content (logos, graphics, text, branding) is Kaecyn Behavioral Health Services property and protected by copyright laws.


11. PROHIBITED USES

14. PAYMENT TERMS & FINANCIAL RESPONSIBILITY

  • All services must be paid at the time of booking
  • 3.5% credit card processing fee applies to all card transactions
  • Membership plans require 6-month minimum commitment
  • No refunds for completed services, membership fees, or early cancellation
  • Late payments will result in service interruption and additional fees
  • All prices are subject to applicable state and local taxes
  • Patient is responsible for all fees, regardless of insurance reimbursement

15. WELLNESS MEMBERSHIP TERMS

  • 6-month minimum commitment required
  • Membership fees are automatically billed according to the selected payment schedule
  • No pausing, freezing, or pro-rating of membership fees
  • No refunds for unused services or early cancellation
  • Cancellation requires a 30-day written notice after completion of the 6-month term
  • Plan changes are only permitted at renewal periods
  • Insurance is not accepted for membership plans
  • Cash pay only for all membership services

16. CANCELLATION & NO-SHOW POLICY

  • No-show fee: Full appointment cost ($200 initial, $100 follow-up, $130 extended)
  • Late cancellation (<24 hours): $75 fee
  • Three missed appointments may result in discharge from practice
  • We reserve appointment time specifically for you

17. FORMS & DOCUMENTATION POLICY

  • Forms/Letters: $150 each (unless specified in membership)
  • Established patients only (minimum 3+ completed visits required)
  • 7-10 business day processing time
  • The clinician has sole discretion to approve/deny requests
  • No forms provided for Long-Term Disability certification
  • No forms for non-established patients
  • Forms requiring extensive evaluation (>10 minutes) are billed at standard rates

18. TREATMENT & DISCHARGE POLICIES

  • Treatment non-compliance may result in discharge
  • Provider may terminate the relationship with a 30-day notice for appropriate clinical reasons
  • Required periodic assessments and follow-ups as clinically indicated
  • Controlled substance policies and requirements as per state regulations

19. INFORMED CONSENT FOR TREATMENT

  • Risks and benefits of treatment will be discussed during intake
  • Alternative treatments available will be presented
  • You have the right to refuse treatment
  • You have the right to seek a second opinion

20. COMMUNICATION POLICIES

  • Email response within 48 business hours
  • Secure messaging preferred for clinical communications
  • After-hours coverage limitations apply
  • Emergency communication protocols are outlined in the intake documents

21. MEDICAL RECORDS

  • Copy fees for medical records may apply per state regulations
  • Records requests processed within 30 days as required by law
  • An electronic health record system maintains confidentiality standards

22. CHANGES TO TERMS
We may update these Terms & Conditions at any time. Changes take effect immediately once posted. Continued site use means acceptance of updated terms.


23. GOVERNING LAW
These Terms are governed by Florida law without regard to conflict-of-law principles.
